What is donor fat? And how it’s used

When it comes to cosmetic procedures, you might be leery of using synthetic fillers or implants. Fortunately, there are other options available, including using your body’s own fat or donor fat from cadavers. Here’s what you need to know about this natural alternative to synthetic fillers.
What is donor fat?
Unlike synthetic options, donor fat is harvested from the body, typically the abdomen or thighs, and can be used to add volume in other locations of the body. Donor fat options and uses can vary, so it’s important to discuss it with your medical team to find the best fit for your preferences.
“Off-the-shelf donor fat products are processed from donated human tissue and require no surgery,” explains Dr. Michelle Lee, board-certified plastic surgeon and founder of PERK Plastic Surgery in Beverly Hills, California. “In contrast, autologous fat transfer uses the patient’s own fat, which is harvested via liposuction from areas like the abdomen, flanks or inner thighs under sedation.”
How is donor fat used?
Donor fat is harvested from the body in a procedure known as “fat grafting” or “fat transfer.” This is done through liposuction and then reinjected into the body, often the face, butt or breasts. If you opt to use donor fat, the procedure doesn’t require harvesting.
“Off-the-shelf donor fat, such as Renuva or Alloclae, is different,” says Lee, referring to fat harvested from cadavers then sterilized. “It is used for smaller, corrective areas where surgical harvesting is unnecessary. It is also an ideal alternative for patients who lack sufficient fat of their own, including those with a low BMI or patients experiencing significant volume loss from GLP-1 medications like Ozempic or Wegovy.” In addition to the donor fat plumping regions without lipsuction or surgery, it helps the body regenerate its own tissue.
Results and what to expect
Donor fat has a long-lasting impact. Unlike synthetic filler, it typically doesn’t dissolve or move because it can establish blood supply in the new location. “Typically, 50% to 70% of the transferred fat survives long-term,” says Lee. “Because it is living tissue, it ages naturally and responds to weight fluctuations, ensuring results look natural for five to 10 years.”
It’s important to ensure that you are a good candidate for donor fat. “Poor candidates include very thin patients who lack sufficient harvestable fat, who are better suited for off-the-shelf donor fat,” she says.
While risks are minimal, autologous fat transfer is still a surgical procedure, so infection and bleeding are possible. Lee says it’s also important to give it time to see the full effect. “Patience is key. Swelling in the initial weeks can make the area look overfilled, followed by a period of reabsorption, with final results taking two to three months to fully settle.”
Moving forward
If you and your medical team decide that donor fat is a good option for you, Lee says it’s important to go in with the right expectations. “The primary misconception is that using one’s own tissue makes the procedure simple or equivalent to a temporary filler; it remains a surgical procedure,” she explains. “Some reabsorption is normal and expected. Setting realistic expectations pre-operatively is critical.”
