Early Sunday morning, on Interstate 93 northbound near Exit 15A, a wrong-way entry set off a chain of impacts that left a 28-year-old man from Haverhill dead.

Riley Sergi was killed when an SUV heading the wrong way collided head-on with his car, the Suffolk County District Attorney’s Office announced Monday. Sergi, authorities said, died after the multi-vehicle crash unfolded as Antone Carvalho, 81, of Somerset, drove into I-93 north from Exit 15B.

Carvalho’s next moves were documented through a preliminary investigation described by police. Officers said Carvalho was driving a Cadillac Escalade when he wrongly entered I-93. As he continued the wrong-way drive. a driver of an Audi A4 and a Honda Odyssey carrying a family of four tried to avoid him and collided with each other.

That collision was followed by a head-on impact, police said. Carvalho continued driving the wrong way and ultimately collided head-on with a Chevrolet Cruze driven by Sergi.

First responders took the family of four to a Boston-area hospital, authorities said. There were no updates on the family Monday, and police did not say whether anyone in the Audi was injured.

The driver of the wrong-way vehicle, Antone Carvalho, is expected to appear in court at a date that has not yet been determined. District Attorney Kevin Hayden said the investigation is ongoing and that charges will be determined.

“I extend my deepest sympathies to the family and friends of Mr. Sergi, and I assure them that the investigation of this tragic crash will be careful and thorough,” Hayden said.
