For the fourth time, President Donald Trump is asserting he got everything right on a cognitive test—this time going even further than before.

In a Truth Social post Saturday, Trump wrote that, “Unlike other U.S. Presidents. none of whom have ever taken an approved. high difficulty. Cognitive Test. ” he scored a perfect 30 out of 30 on the test. calling it “extreme intelligence.” He then immediately turned to politics. writing. “Are the Dumocrats really surprised?” and demanding that people running for president and vice president be forced to take similar tests. He added that Congress and Democrats “should demand it!”.

Trump said the test was not a one-off. He wrote that this was his “fourth such test. ” that all were “PERFECT. ” and that he has logged “120 correct answers out of 120 questions asked.” He said it is “very rare that anyone gets a Perfect Score. ” and that it is even rarer when it happens “four times in a row.”.

The most recent test took place May 26, during his annual medical examination at Walter Reed National Military Medical Center. Trump’s test was the Montreal Cognitive Assessment, or MoCA. Doctors use the MoCA to identify signs of cognitive decline and dementia.

The White House released a memo Friday recapping results of Trump’s medical examination. The memo says Trump’s “cognitive function” was assessed with the MoCA and found to be “within normal limits” with a perfect score.

Still, Trump’s framing—equating a perfect MoCA result with “extreme intelligence”—is where his claims collide with medical reality.

Medical experts have said the MoCA does not test IQ. And on Friday, Dr. Jonathan Reiner, a CNN medical analyst, pushed back on Trump’s interpretation and the significance of repeating the test. Reiner told CNN’s Abby Phillip that the MoCA is “not really that useful when you take it over and over and over again. because the questions don’t change that much.” He added that “Once you’ve taken it now four times. it’s probably not that difficult to pretty easily get through it.” Reiner emphasized the test’s purpose by saying. “It shouldn’t be hard to get through it anyway. because it’s a dementia screening test. it’s not an IQ test.”.

In a Saturday X post, Reiner also questioned why Trump was given another exam so soon, given that he’s had three scheduled medical appointments at Walter Reed in the recent past, according to ABC News reporting.

The timeline is now central to the debate: Trump took his latest MoCA on May 26 as part of an annual checkup. the White House memo described his cognitive function as within normal limits. and Reiner argued that repeating the same dementia-screening test multiple times can blunt the meaning of a “perfect” result.

Taken together, the dispute isn’t about whether the MoCA result was recorded—it’s about what that score is supposed to show, and what Trump chose to claim it proves.

  2. Wait MoCA is like a memory test right? If he got perfect four times then wouldn’t that mean he’s totally fine forever or something? I don’t get why people keep bringing up mental acuity if it’s always perfect.

  3. This is proof the test is rigged? Like if Walter Reed just gives him the answers or whatever. Plus “Dumocrats” like come on… who cares about 120 correct answers when he can’t even spell stuff right on TV half the time.
