Saturday’s Independence Day celebrations may be a cultural milestone on the calendar, but the weather is not cooperating. An intense heat wave is settling over the East and Central U.S., exposing millions to temperatures above the seasonal average and putting them at risk of heat-related illness.

Across up and down the East Coast and the Central U.S., the mercury could surpass 100 degrees Fahrenheit (38 degrees Celsius). In some places, such as New York City, daytime temperatures could feel as hot as 115 degrees F (46 degrees C). For many people, staying safe is about doing the basics—cooling down quickly and not pushing the body too far. But there’s another danger hiding inside well-meaning habits: hyponatremia. low sodium in the blood. which can be triggered when people drink too much water.

Most public health messaging during a heat wave spotlights two conditions: heat exhaustion and heat stroke. Heat stroke is the more serious one. and it can happen when the body’s internal temperature rises above 104 degrees F (40 degrees C) or when someone becomes extremely dehydrated. It can be fatal without immediate medical attention.

There’s also hyponatremia—often barely mentioned in heat-related guidance. The condition translates to low sodium in the blood and is often linked to drinking too much water. People can confuse early hyponatremia symptoms with heat exhaustion. and then unintentionally make the problem worse by drinking more water to cool down.

Your body’s salt-water balance is crucial for the proper function of cells. When salt levels in the blood take a plunge, cells swell up. In its early stages, hyponatremia is often mistaken for heat exhaustion. But that extra water can worsen the swelling, quickly affecting other body parts—including the brain. That can lead to loss of consciousness, seizures, and in some cases, death.

The CDC’s heat guidance talks clearly about drinking enough to avoid dehydration. but it also contains a warning that gets missed in the rush of a heat wave: drinking more than 48 ounces of liquid in an hour could cause salt levels to dip to dangerously low levels. The word “liquid” matters here. It can mean water, fruit juice, tea, soda, or any number of other drinks.

Know the signs of heat illness

Heat exhaustion is the first step toward heat stroke. It’s caused by overheating and dehydration—specifically, the loss of salt and water through sweat. People with heat exhaustion typically feel dizzy, start sweating profusely, have a fast pulse, and can feel sick.

Heat stroke is the more dangerous condition. If heat exhaustion is left untreated, it can spiral into heat stroke—a potentially fatal condition. People with heat stroke can feel confused, be unable to speak properly, and become agitated. Some breathe heavily, notice their heart rate going into overdrive, and can even lose consciousness. Unlike the earlier stage, they might not be sweating at all.

If untreated, the extreme heat inside the body can cause cells to essentially “cook,” leading to organ failure, brain swelling, and often, a heart attack.

Hyponatremia can also be mistaken for other heat illness early on. It isn’t always caused by heat itself. but it can show early symptoms such as muscle cramps. nausea. headache. and tiredness. If it’s untreated—or if people drink more water trying to ease symptoms that feel like heat exhaustion—the signs can become more severe. People can suffer seizures, feel confused or have hallucinations, or fall into a coma. Within just hours of developing the condition. hyponatremia can cause the brain to swell so much that it herniates. pushing the swollen brain stem into the spinal cord. which is often fatal.

Do drink water—but not too much

The U.S. Centers for Disease Control and Prevention (CDC) recommends that people drink water to stave off dehydration. For people working outside or exerting themselves in the heat, the CDC says to drink eight ounces (a cup) of water every 15 to 20 minutes.

For an eight-hour workday, that timing would equate to drinking at least 1.66 gallons, or more than 6 liters, of water. The CDC also warns that thirst is a poor marker of dehydration.

Still. some physiologists and wilderness medicine experts argue that certain advice isn’t always helpful because it can lead to overhydration. The CDC’s own limit is direct: drinking more than 48 ounces of liquid in an hour could cause salt levels to dip to dangerously low levels. And to be truly hydrated. the CDC notes that people need to eat regularly in the heat to replenish salt levels.

A sports drink such as Gatorade isn’t the quick fix many people assume it is. Off-the-shelf sports drinks typically contain less than 20 percent of the recommended amount of salt an adult should consume in a day. which means they may not be salty enough to raise blood salt levels. Eating a one-ounce serving of salted pretzels is cited as a better bet.

In extreme cases of hyponatremia, doctors feed patients extremely salty solutions, such as a half-cup of broth made with four standard beef bouillon cubes, because that amount of salt is much more effective in raising salt levels back to where they need to be.

Cool down your body before it’s too late

Prevention is the point where people still have options. The simplest step is to remain indoors in air-conditioned spaces with easy access to cold water—for drinking and for showering or wetting towels to cool the body. That won’t work for everyone, so the guidance expands.

Staying inside can mean keeping windows and blinds closed—light means heat—in an adequately air-conditioned space. Some cities offer cooling stations where people can access air conditioning even if they don’t have it installed at home. The National Center for Healthy Housing has a state directory to help people find a local cooling center.

If you don’t have air conditioning but you do have fans, they can help under certain conditions. If the air outside your home is cooler than it is inside. position a fan by an open window and point it inward. If possible, place yourself in front of the fan. Wet clothes can make the cooling more effective.

Sometimes, turning a fan outward at night—when temperatures naturally drop—works better. The idea is to push hot air out of the room and pull cooler air in through the open window.

For anyone who does go outside, protective steps stack up: wear a hat, loose clothing, and sunscreen, and reapply regularly. Stay in the shade as much as possible and bring a handheld fan if you can. A cold. wet face towel or an ice pack placed on the neck or pulse points can help the body stay cooler.

Avoiding alcohol and caffeine can also help the body maintain its internal temperature regulation.

Some research suggests that soaking your feet in a bath of ice water can help cool the body’s core. The advice also points to other cooling methods: taking regular cool showers and baths. wetting clothes or towels with cold water. and putting pillowcases in the fridge or freezer can help cool the body down and hopefully support a better night’s sleep.

The practical takeaway is straightforward even if the weather isn’t: in a heat wave. dehydration is dangerous—but so is drinking too much. Heat exhaustion and heat stroke can escalate quickly, and hyponatremia can look like the wrong problem early on. With temperatures near and above 100 degrees Fahrenheit across huge swaths of the country. the safest plan is to cool down early. drink with restraint. and watch for the warning signs before heat becomes an emergency.
