The wait is over: Taylor Swift’s “Patient Zero” music video is here!
The pop star gave Swifties something to celebrate with the release of the music video for her latest single, starring Dakota Johnson and Colin Farrell. She premiered the music video Sept. 27 at the 2026 MTV Video Music Awards at the Peacock Theater in Los Angeles.
The cinematic video opens in a snowy cemetery, where Johnson approaches a gravesite, and Swift appears behind her wearing the same outfit. From there, Johnson finds herself trapped in a toxic relationship with Farrell, who controls, belittles and gaslights her as the video begins to reveal that Swift’s character lived through the same nightmare before her.
The connection becomes clear in the final chorus: Farrell has been secretly poisoning the women in his life. A newspaper clipping reveals Swift’s character was a wealthy heiress who died from a mysterious illness, leaving her husband to inherit billions.
“Patient Zero” is one of four new tracks that appear on the deluxe edition of Swift’s 12th studio album, “The Life of a Showgirl: The Encore.” The record also includes “Cleveland!,” “Pink Clouding” and “Babylon.” And in true Swift fashion, the video is packed with Easter eggs for fans to decode.
Johnson introduced Swift at the Sept. 27 MTV Video Music Awards, where the singer-songwriter accepted the inaugural Artist Director Honors award. After her acceptance speech, Swift tossed to the world premiere of “Patient Zero,” her latest work behind the camera.
After the premiere, Swift explained the cinematic recipe behind “Patient Zero” in a post on X.
“I got to explore the cinematic and literary genres I love so much,” Swift wrote, saying she wanted to blend “gothic ghost stories, true crime, psychological thrillers” in a setting unlike much of her previous work.
The stark house where Johnson’s relationship with Farrell unravels was intentional. Swift said it served as “a metaphor for the coldness of a relationship that’s faded and atrophied.”
Swift also thanked cinematographer Emmanuel Lubezki, production designer Ethan Tobman and editor Chancler Haynes, along with Johnson and Farrell. She revealed Cara Delevingne plays “the lusty other woman” and joked about spending the shoot twinning with her real-life friend Johnson.
“And lastly, thank you to the VMAs for giving us a World Premiere at the place where celebrating music videos became a thing,” Swift wrote.

2026 is the year of pop-up performances
Although Swift hasn’t performed at the VMAs since 2019, she has made several onstage appearances in 2026.
At the June 9 “Toy Story 5” premiere in Los Angeles, the curtain rose after the movie with Swift seated at a piano. She performed “I Knew It, I Knew You” before Randy Newman joined her for “You’ve Got a Friend in Me.”
Two weeks later, Swift made a surprise appearance at Tight End University’s “Tight Ends & Friends” concert at The Pinnacle in Nashville. Lainey Wilson introduced Swift, who performed “Love Story” after telling the crowd it was a special request from San Francisco 49ers tight end George Kittle.
Swift returned to Los Angeles in August for a Recording Academy event at the Grammy Museum, where she opened with a three-song mashup of “I Knew It, I Knew You,” then moved into “August” and “All Too Well.” She also sang at a separate intimate event for Emmy voters that month, though she did not disclose the songs she performed.
Swift entered the VMAs with nine nominations, second only to Madonna’s 11. The singer-songwriter-director’s history of performing at the VMAs stretches back 17 years.

- 2010: Swift debuted “Innocent,” one year after Kanye West interrupted her acceptance speech.
- 2012: Swift closed the show with “We Are Never Ever Getting Back Together” from “Red.”
- 2014: Swift performed “Shake It Off” weeks after announcing “1989.”
- 2015: Swift surprised viewers by joining Nicki Minaj during the show’s opening performance. The two did a mash-up of “The Night Is Still Young” and “Bad Blood.”
- 2019: Swift opened the show with a colorful performance of “You Need to Calm Down” before picking up a guitar for “Lover.”
