On a recent episode of the Derm Approved podcast, the former Playboy Bunny sat down with Dr. Sheila Farhang and broke down the “nips and tucks” she’s had since having babies—starting with the lower facelift she says she feels “really happy” about. “I’ve had a lower facelift on my jawline. I’m really happy I got it done,” she told the doctor.
The decision. Madison explained. came after her second pregnancy left her with changes she didn’t feel she could fix through less invasive options. “I just felt like after having kids. you know. I gained a lot of weight from my second pregnancy and there was just nothing I could do. There were no lasers or anything that could really do what I wanted it to do so I found somebody whose before and afters I really loved.”.
Madison, 46, said she’s had “a little bit of lipo” under her chin, with her main focus placed on her jowls and tightening her lower face. She linked that work to becoming a mother to daughter Rainbow in 2013 and son Forest in 2016 with ex-husband Pasquale Rotella.
She also made it clear she wasn’t looking for a dramatic transformation. “I didn’t want to go too far. ” she said. describing how she searched the web until she found a doctor with results she liked. When she went in, she chose what she called a “conservative” approach. “I’d rather have it not go far enough and then I can go back and maybe do more rather than have it go too far and I’m looking like the Joker.”.
Recovery didn’t come with a glamorous preview, either. Madison said she kept quiet about the process beforehand because she’s “superstitious. ” and she described the aftermath honestly: “So she took about two weeks off from her podcast to let herself heal in private. And when the swelling went down. the scars persisted.” She added that she isn’t bothered by the visibility of those scars. saying. “If you look close. you can totally see it. but I don’t care. I’m kind of open about it. I already talked about it on my podcast.”.
That lower facelift wasn’t her first trip to the cosmetic surgeon. Madison noted she’s previously had work done on her breasts and had a nose job. Now, she says she’s “playing around with hand rejuvenation,” signaling the next area she’s curious about.
The new honesty lands with extra weight in light of how her family has reacted to her appearance. In 2023. she spoke to People about her kids roasting her for trying to keep up—specifically for “trying so hard” to maintain her look. She said she’s “very relieved” because her daughter and son roast her for dyeing her hair. having extensions. and wearing a lot of makeup. Madison added: “So I think I’m good. Knock on wood. I think they’re not going to fall into the trap of trying to make themselves look over-processed or trying too hard for the beauty standard because they are over it.”.
Madison’s latest comments leave one theme consistent: she wanted results she could live with—measured, not extreme—and she’s willing to talk about the parts people usually keep private, from swelling to scars.
