Jose Berrios’ 2026 season debut has dragged on longer than Toronto hoped, and now the Blue Jays are openly weighing “surgery is on the table,” with coach John Schneider addressing the fallout from a lingering elbow problem.

Berrios has yet to pitch for the Jays this season.. After four minor-league rehab starts, the right-hander looked closer to returning, only to run into new elbow inflammation.. His most recent start came with the Buffalo Bisons on May 3. where he struggled through 3.2 innings and allowed seven runs. including a grand slam.

On Tuesday, Berrios met with Dr. Keith Meister, described as one of the top elbow surgery doctors, and discussions are continuing about what the next steps will be.

The uncertainty comes against a backdrop of a difficult 2025 finish. Berrios made 31 appearances last season, posting a 4.17 ERA with 138 strikeouts, but he was left off the post-season roster after struggling in the second half. He also was placed on the injured list before the playoffs began.

With the Jays set to begin a three-game series in Detroit against the Tigers, Schneider turned attention to other roster questions. He said his starter for Saturday is still to be determined, but Spencer Miles figures to be part of the plan.

Alejandro Kirk, who has been out since April 3 with an injured thumb, is also moving in a positive direction. Schneider said Kirk is throwing and hitting again, and the all-star catcher’s thumb is “responding well” as he looks toward returning to catching pitches.

Schneider added that Addison Barger is “doing beter” and is hoping to return to hitting and throwing next week. Max Scherzer is back to throwing, and Yimi Garcia is slated to throw a bullpen on Tuesday.

For a team trying to steady its rotation and get key bats and glove work back in rhythm, the next medical decisions around Berrios could shape the timeline of Toronto’s season far more than any single game in Detroit.
